However, this method leaves it completely up to chance, and unfortunately, the odds aren’t that great. Players have gone on to open dozens of Uncertain Heart Caches without even seeing a Wrathful Malingnat Heart, much less the specific one that they need.

Thankfully, there are useful methods that players have used to increase their odds of obtaining Wrathful Malignant Hearts to help them farm the Creeping Death Heart in Diablo 4.

  • Method 1 – Farm Malignant Tunnels in World Tier 4. Malignant Tunnels can potentially contain a Wrathful Malignant enemy that will guarantee a Wrathful Heart drop.

  • Method 2 – Summon a Wrathful Malignant Elite from a Wrathful Malignant Outgrowth.
    • You’ll need a Wrathful Invoker, which you can obtain as a drop from in-game activities or by chance through the Uncertain Invoker at Cormond’s Workbench.
    • Players will need to head to the end of Malignant Tunnels and find a Wrathful Malignant Outgrowth to use the Wrathful Invoker on. It seems like leaving a lot to chance, but you’ll find Invokers and Outgrowths frequently in your adventures.
  • Method 3 – Craft a Caged Heart of the Barber using an Uncertain Heart Cache.
    • Use Cormond’s Workbench in Kyovashad to craft Uncertain Heart Cache for the chance to obtain a random Malignant Heart, including the Caged Heart of Creeping Death.
    • Only use this method if you have an intense amount of extra materials because the chance of getting a Wrathful Malignant Heart is very low.

  • Method 4 – Farm the Caves of Velkhova Dungeon.
    • This dungeon is only available to players that have not completed the quest, The Cold Hard Truth.
    • You want to run this dungeon multiple times to increase the probability of running into Wrathful Malignant enemies.

We recommend using Method #1 because it allows you to farm XP, materials, and complete Season Journey challenges while also giving you a chance to get the Caged Heart of Creeping Death.

The Caged Heart of Creeping Death will increase the potency of your Damage Over Time effects by (32-42%) for each different Crowd Control effect on the target. Unstoppable enemies and Staggered bosses instead take (114-134%) increased damage from your Damage Over Time effects.
