The Fleshrender is a Unique Two-Handed Mace exclusive to the Druid class and was released in Diablo 4’s big 1.1 patch for Season 1.

As a Unique Item, the Fleshrender is among some of the rarest and most powerful items in the game.

How to Get Fleshrender in Diablo 4
To get the Fleshrender, you’ll need to be playing Diablo 4 in either World Tier 3 or World Tier 4 difficulty. Only then will the Fleshrender be able to drop as loot from defeating enemies and opening chests.
You cannot access Unique Items in World Tier 1 or 2.
To access World Tier 3, you must complete the Cathedral of Light Capstone Dungeon located in Kyovashad.
For World Tier 4, you need to complete the Fallen Temple Capstone Dungeon in the Dry Steppes region while playing in World Tier 3.

Once you have chosen to play in either World Tier 3 or 4 and have met the above requirements, you’ll have to simply play the game and hope the Fleshrender drops as loot.
Like with all other loot in Diablo 4, Unique Items will drop through random chance. It’s important to note that their drop rate is much lower, with some players reporting a 1 in 600,000 chance of seeing them appear.
Fortunately, there are a few activities you can take part in to somewhat increase your chances of seeing the Fleshrender drop in Diablo 4:
- Play in higher World Tiers
- Defeat World Bosses
- Open Reward Chests and Caches
- Complete Whispers of the Dead Requests
- Take part in Helltides and open Helltide Mystery Chests
- Complete Nightmare Dungeons (higher tiers have higher drop rates for loot)
- Defeat Elite enemies
- Take part in Legion Events

Even though the above activities will help your chances, don’t get your hopes up! There’s a reason these are called ‘Unique Items’ after all.
What is Fleshrender Diablo 4?
The Fleshrender is a Unique Two-Handed Mace that is exclusive to the Druid class in Diablo 4. It has the unique effect of adding significant damage buffs to the Druid’s Debilitating Roar and Blood Howl abilities while enemies are poisoned.

The Fleshrender Unique Mace compliments the Druid’s tankiness and survivability really well. It gives significant boosts to Defensive Skills and well as increasing your damage while Healthy.
However, the Fleshrender’s main draw is its buff to poison-focused Druid builds. Not only do you deal more damage to poisoned enemies, but this is also supplemented by dealing big damage to nearby poisoned enemies when using the Druid’s Debilitating Roar and Blood Howl abilities.
