It has D Scaling with DEX and STR as well as a powerful weapon skill, Shriek of Milos – a scream that reduces damage negation and resistances for nearby enemies. However, it is the hidden ability that makes it so powerful!
While the Sword of Milos is in your hand, you gain 5 Focus Points for every enemy you kill, making it a great weapon for spellblades or a secondary sword for anyone who uses a weapon with a powerful skill. This is because you can gain loads of FP very quickly without using up a flask.

How to Get the Sword of Milos in Elden Ring

- You can get the Sword of Milos by killing Dung Eater in the Subterranean Shunning Ground. However, this will mean you can’t complete his questline.

- Alternatively, you can continue the questline by releasing Dung Eater’s Body from the cell in the Shunning Grounds, reading his message in Roundtable Hold, and then going to the moat outside Leyndell.
- There you can fight the Dung Eater NPC Invader and get the Sword of Milos by defeating him.

As some others have mentioned, it is likely a lot easier and a lot faster to get the Sacrificial Axe, which has the exact same passive ability as far as I am aware. One can find it after defeating a Deathbird, an optional world boss, just Northwest of Morne Castle in the southern most part of Limgrave. It is still a very low level area and you can very likely go down there before you even fight Margit. Just make sure it is night time for the Deathbird to spawn. I would recommend fighting it on horseback or even bringing a friend, but overall, it isn't that tough a boss to figure out. Either way, Dungeater or Deathbird, happy hunting.

Could always just get the Sacrificial Axe from the deathbird north-west of Castle Morne, no need to progress most of the way through a mid game quest.
