The Barbarian class currently has 10 exclusive Unique Items it can equip in Diablo 4. In detail, these 10 pieces include 6 Unique Weapons, 3 Unique Armor pieces, and a Unique Amulet.

The Rage of Harrogath’s cooldown reduction of your Non-Utimate skills is brilliant for every build. It’s also really easy to proc, as there are many ways to inflict bleed on your enemies as a Barbarian.
It also applies a hefty Crit Chance while fighting against Elite enemies, which is great for farming Nightmare Dungeons. An easy choice.
A-Rank
- Ramaladni’s Magnum Opus
- Strengths: Buffs damage to all skills when using the weapon
- Weaknesses: Build-specific, other weapons may be better for certain skills
The increased damage buff is a very nice feature of Ramaladni’s Magnum Opus, even though it is dependent on how much Fury you have. When added up, this can result in a significant boost to your damage output even despite the Fury drain every two seconds.
Despite this, the damage increase only applies when using skills while wielding the Magnum Opus, meaning that you’ll have to build with this in mind to make the most of it. There may be better alternatives out there for certain skills, but this is still a decent choice for raw damage output if built correctly.

B-Rank
- Azurerath
- Strengths: Great for survivability, fun to use
- Weaknesses: Lower-than-average DPS
Azurerath is one of the new Unique items released for Diablo 4’s Season 1. It has a fun unique effect of giving your Core Skills a chance to freeze enemies on a Lucky Hit. This is great for running Nightmare Dungeons – it can do a lot to increase your survivability.
However, its DPS leaves a little to be desired and you’ll likely find other swords that have a much higher damage output. Its affixes are also missing some key stats, such as Crit and Vulnerable Damage, which you can also find elsewhere.

- Fields of Crimson
- Strengths: Good for builds focusing on bleed damage
- Weaknesses: Focus on Rupture
If you’re running a bleed build, the Fields of Crimson is a decent option for your Barbarian. It gives a pretty hefty boost to the Rupture ability, causing bleeding damage over time to your targets.
It also creates a blood pool when the ability is activated, causing enemies to take even more bleeding damage when standing in it. However, it’s not a good item if you don’t want to focus on Rupture.
- Gohr’s Devastating Grips
- Strengths: Decent buff to Whirlwind
- Weaknesses: Bad after nerf, no Crit Chance
There was a time when Gohr’s Devastating Grips reigned supreme. Its unique effect gives a sizable buff to the Whirlwind ability, causing a fire explosion on use.
Unfortunately, the item has been heavily nerfed since launch. A damage cap has been applied to this unique effect, meaning that the item is a lot less valuable than it was before. It also does not have Crit Chance, which is a really important stat for the Whirlwind ability.

- Overkill
- Strengths: Buffs Death Blow ability, fun to play
- Weaknesses: Poor stats, nerfed
Overkill is a really fun weapon, primarily because of its unique effect. It adds an AOE shockwave to your Death Blow ability and will reset its cooldown for any enemies who die in the blast.
However, the item was nerfed by removing its ranks to Death Blow, making it less useful overall. Its base stats are nothing to write home about either.
C-Rank
- 100,000 Steps
- Strengths: Good for gaining Fury when activating Walking Arsenal Key Passive
- Weaknesses: Weak unique effect, no movement speed bonus, very niche
The unique effect of 100,000 Steps is very build-specific, meaning you’ll need to receive all three damage bonuses from the Walking Arsenal Key Passive to pull it off. When you do, you’ll proc a Ground Stomp and gain a nice chunk of Fury every 30 seconds.
However, this locks you into a very specific playstyle for an effect that isn’t worth all the effort. You’ll also be missing out on a bonus to Movement Speed, which is one of the more important affixes to look out for on boots
- Ancient’s Oath
- Strengths: Buffs Steel Grasp
- Weaknesses: Focus on Steel Grasp, poor stats
The Ancient’s Oath gives a decent buff to the Steel Grasp ability, allowing you to pull in more enemies to deal damage up close.
There are just simply better skills to focus on other than Steel Grasp. You’d fare much better to replace it with something like Death Blow – doing so eliminates the usefulness of Ancient’s Oath.

- Battle Trance
- Strengths: Increases Attack Speed,
- Weaknesses: Focus on Frenzy, misses out on powerful Legendary Aspects
Battle Trance provides a decent buff to your Barbarian’s Attack Speed when you have maximum stacks of Frenzy. The extra Attack Speed is good, though the Amulet’s focus on the Frenzy Basic Skill is what lets it down. There are better Basic Skills to run for the Barbarian.
If you equip Battle Trance, you’ll also miss out on other Rare and Legendary amulets with amazing stats and the flat 50% bonus to Legendary Aspects.
- Hellhammer
- Strengths: Buffs burning damage
- Weaknesses: Lackluster unique effect
The Hellhammer gives some benefit to those building around burn damage but is lacking in pretty much every other department.
The Upheaval Skill that the item is based on is quite underwhelming, especially when coupled with its unique effect of igniting the ground on use. There are far better options and skills out there for Barbarians.

How to Get Unique Items in Diablo 4
To get Unique Items in Diablo 4, you need to be playing in either World Tier 3 or 4. Unique Items can only drop as loot in these difficulty Tiers.
To access World Tier 3, you need to complete the Cathedral of Light Capstone Dungeon in Kyovoshad.
For World Tier 4, you need to complete the Fallen Temple Capstone Dungeon in the Dry Steppes region, while playing in World Tier 3.
As with any other loot in Diablo 4, the drop rate for Unique Items is based on random chance, meaning that you’ll have to play the game as normal and hope to see them as loot.
Unique Items are the highest rarity tier of loot in Diablo 4. Some players have reported a 1 in 600,000 chance of seeing them drop. Suffice it to say, you’re going to have to rely on a bit of RNG to get your hands on them.

With that said, engaging in the following activities should help to increase your chances of getting Unique Items in Diablo 4:
- Play in higher World Tiers
- Engage in Helltides and open Tortured Gift Chests (particularly Tortured Gift of Mystery Chests)
